What's going on with Slow Early Termination Fee Reimbursement?

I've moved to T-mobile late September from Verizon.

I had 2 lines that had ETF of $510 total. Submitted the form as soon as possible.

When I checked with the tracker, it says Received 10/13, Approved 11/12 with below saying "We've approved your reimbursement application. You should receive your reimbursement within 2-3 weeks."

 

It's now 5th week from approved status and I haven't received the reimbursement. Nor it highlights the 3rd step "Mailed"

 

I have called CS and received no satisfying response. They just read off the reimbursement tracker and thats it.

 

Wish someone could help me on this issue.
